Treatment Screening Questions

I give consent to Advanced Mobile IV to administer vitamins, minerals, medications, and other nutrients via injection and/or intravenously. I have been informed of possible risks including discomfort, bruising, infection, bleeding, allergic reaction, and other complications. I understand this treatment is voluntary, I am 18 or older, and I am responsible for payment at the time of service.
